Overview / Description:

In this group research project, students will select an industrial accident to research.  They will also examine the OSHA regulations which weren't followed which could have contributed to the accident, as well as brainstorming possible ways the accident could have been avoided. Finally, students will find three workplace environments to examine using a Safety Quiz.

Learning goals/objectives:

After completing this activity, students should be able to . . .

  • research industrial accidents.
  • understand how adhering to OSHA regulations can avoid accidents.
  • examine ways that the accident chain can be broken.
  • examine workplace environments for safety.

In small groups:  Students will select an industrial accident to research.  For a list of accidents to choose from, see the website "Deadliest Workplace Accidents" at

As each group signs up for a different accident, they should take notes on the following questions:
  • What happened during this accident?
  • Where and when did this take place? 
  • How many lives were lost?  
  • How many people injured?

Next, using the current OSHA Standards,  groups should find 3 OSHA regulations that were broken to cause the recorded industrial accident.  OSHA Safety Standards can be found here:

Groups should brainstorm and find at least three answers to the question "How could the accident have been avoided?"SM15 min.
Students will individually select 3 work place environments to take Safety Quiz from the website:

Students should choose safety quizzes appropriate to the work environment they have selected (for example, not completing the "Working Safely with Radioactive Materials" quiz if the environment does not work with radioactive materials at all). If the student is visiting a workplace outside of school, they may work with an industry mentor on these quizzes.

Students should print their completed safety certificates for each of the safety quizzes to document their scores.   I 10 min.

Summative Assessment - Workplace Safety Group Worksheet and individual safety quiz certificates

Wrap-Up: Class discussion on the importance of workplace safety. Teacher should encourage students to share what they found as they completed their safety quizzes.

Extension Activity (for intervention or enrichment):

  • Groups could create presentations on their researched industrial accidents
  • Teacher could lead entire class in brainstorming possible solutions to the industrial accidents presented
  • Students could be matched with area businesses and assigned a workplace mentor at the business prior to investigating the accident quizzes to get real-world feedback on the importance of workplace safety
  • An industry partner could be invited in to discuss local industrial accidents and their implications
