These documents were created to help determine if an EL student has a language difference versus a disorder and to help assess and analyze the learning environment for English learners who may be struggling in school. They include a parent interview, a language background form, an Exclusionary Factors and an observable behaviors template.

 

They were adapted and created in Google forms with the permission of Courtney Seidel, MS, CCC-SLP is an Associate Clinic Professor at the University of WI Madison in the Department of Communication Sciences & Disorders.
